You rolled your ankle, it swelled up fast, and now every step on uneven ground feels risky. Most ankle sprains respond well to guided rehabilitation, and the sooner you start, the better.

A sprained ankle happens when the ligaments that hold your ankle bones together are stretched or torn, usually when your foot rolls inwards. You get pain, swelling and bruising, and the ankle often feels unstable for weeks afterwards.

Rest alone is rarely enough. Ligaments heal, but the balance and strength systems around the joint need retraining, which is why so many people sprain the same ankle again.

What is a sprained ankle?

A sprain is damage to a ligament, the tough band of tissue that connects one bone to another. In the ankle, it usually happens when your foot turns under you faster than your muscles can react, overstretching the ligaments on the outside of the joint.

The tissue tears to some degree, bleeds, and swells. Swelling and pain settle over the following weeks, but the nerve endings inside the ligament that tell your brain where your foot is in space stay affected for longer. That is why the ankle can feel unreliable long after the bruising has gone.

Things that make a sprain more likely include:

  • A previous ankle sprain, especially one that was never rehabilitated

  • Sports with cutting, landing and contact, such as netball, football and basketball

  • Reduced ankle and calf strength, or poor single-leg balance

  • Worn, unsupportive or ill-fitting footwear

  • Walking or running on uneven surfaces, trails or sloped ground

Your plan comes out of your assessment: which ligaments are involved, how much they are loaded, and what you need to get back to.

Protection and early loading

short-term strapping, bracing or crutches where needed, then graded weight bearing, because controlled load helps ligaments heal in a stronger alignment

Swelling and pain management

compression, elevation and hands-on techniques to reduce swelling so you can move the joint sooner

Joint mobilisation

manual therapy to restore ankle bend, which you need for stairs, squatting and running

Strength work

loading the calf, peroneal and hip muscles that control the ankle when you land or change direction

Balance and proprioception retraining

single-leg and reactive drills that retrain the nerve signals from the ligament, reducing the chance of rolling it again

Return to sport progression

staged running, hopping, cutting and landing work until your ankle handles the demands of your sport

Your physiotherapist checks the ligaments, joint movement, strength and balance, and screens for a fracture or a high ankle sprain.

A clear plan

You leave knowing what the injury is, what to do at home, and what the next few weeks look like.

Review and adjust

We reassess as your ankle improves and progress the load so you are not stuck doing the same exercises.

When to book an appointment

Book an assessment if:

  • Your ankle is still painful or swollen more than a week after the injury

  • You cannot walk normally, run, or return to training

  • Rest, ice and strapping have not made a real difference

  • You are not sure how bad it is, or you have sprained this ankle before
