You have rested it, stretched it, changed your shoes, and months later the pain is still there every morning. Shockwave therapy is a non-invasive treatment for tendon and fascia pain that has not responded to the usual approaches.

Shockwave therapy uses acoustic pressure waves delivered through the skin to stimulate a healing response in tendon, fascia and bone. It is most often used for pain that has lasted several months and has not improved with exercise, footwear changes or orthotics.

It is not a stand-alone fix. Your podiatrist uses shockwave alongside a loading program, footwear advice and activity changes, so the tissue is being repaired and retrained at the same time.

Who does it affect?

What is shockwave therapy?

Shockwave therapy, also called extracorporeal shockwave therapy or ESWT, sends high-energy sound waves into a specific area of tissue through a handpiece pressed against your skin. Nothing is injected and nothing is cut.

Long-running tendon and fascia pain is usually not simple inflammation. The tissue has become degenerated, with disorganised fibres and a poor blood supply, which is why it stalls and stops improving on its own. Shockwave works by deliberately restarting that stalled repair process.

  • Increases blood flow: the waves trigger new small blood vessel formation in tissue that has a poor supply.

  • Stimulates cell activity: treated tissue produces more collagen-building activity, which supports repair.

  • Breaks down calcification: repeated pressure waves can help disrupt calcium deposits and thickened scar tissue.

  • Reduces pain signalling: the treatment alters how nerve endings in the area transmit pain.

A course of shockwave sessions

treatment is applied directly over the painful tissue, usually across several sessions spaced about a week apart, to build the repair response over time.

A progressive loading program

targeted strengthening reorganises new tissue along the lines of load, so the tendon or fascia tolerates walking, standing and running again.

Footwear and orthotic therapy

changing how force passes through the foot reduces the strain that caused the tissue to break down.

Activity and load management

adjusting training volume, work shifts or surfaces keeps the tissue below its irritation threshold while it repairs.

Review and adjust

your response is reviewed as the course progresses, and your loading program and footwear are adjusted as your pain settles.
